MACIPA’s social work department provides additional support services to patients with the most complex needs. The goal of the social work department is to help maintain patients in the community as long as possible. When MACIPA physicians or nurse case managers identify, for example, that patients may have issues with safety, complicated family problems or difficulty connecting to programs and services in the community, they can ask one of the social workers to be part of the caregiver team, bringing additional expertise to patient care. The MACIPA social workers are available to make a home visit or provide assistance by phone to help resolve problems. They will also provide assistance to a patient’s caregivers who is experiencing stress. They maintain relationships with social service agencies in the community and know which resources will be most beneficial.
