MACIPA Launches the Serious Illness Conversation Initiative

MACIPA has been accepted to join the Serious Illness Care Program of Ariadne Labs, in a new approach to transform the care of patients with serious illness. MACIPA will join a small cohort of leading health systems as early innovators.This initiative will support clinicians to implement a systematized clinical workflow for conversations with seriously ill patients about their goals and wishes for health care.

Serious illness conversations about patient values and goals have been shown to improve quality of care, reduce hospitalizations, and improve mental health outcomes.  However, more than 70% of clinicians don’t have a system in place to routinely ask about goals of care1.

MACIPA’s Serious Illness Care Program implementation team will support sites with patient identification, Epic integration via Smartform, development of clinical workflows, training, and ongoing coaching for all practice staff. Serious Illness Care Program follows MACIPA’s 2016 quality forum on “Communication in Serious Illness” and the 2018 forum on “Conversations through the Continuum”. Serious Illness Care Program is supported by funding from Blue Cross Blue Shield of MA.
