Pharmacy Program Overview

Our Pharmacy Program is designed to enhance patient care through expert medication management. A dedicated Clinical Pharmacist provides support to all clinicians, while being embedded in five primary care practices.  She works closely with physicians, nurse practitioners, physician assistants, nurses, and support staff.  Together they ensure that patients receive safe, effective, and affordable medication therapy.

Core Services

Medication Safety & Optimization

Identifying potential drug interactions, supporting deprescribing efforts, and managing polypharmacy.

Improving Adherence

Promoting consistent medication use through education and personalized support.

Formulary & Cost Support

Answering coverage questions, recommending therapeutic alternatives, and connecting patients with cost-saving resources.

Proactive Medication Reviews

Reviewing medications ahead of office visits for patients with diabetes, hypertension, or those enrolled in the MA Shared Savings Program.

Collaborative Care

Participating in PCP pod meetings to share insights and address emerging medication-related issues.

Barrier Resolution

Working directly with patients and providers to address challenges such as affordability, side effects, or complex regimens.

Our Commitment

The goal of the Pharmacy Program is to help patients live healthier, more empowered lives. Our Clinical Pharmacist is also available for comprehensive medication reviews, especially for patients with uncontrolled chronic conditions or gaps in care—always guided by evidence-based practices.
