Developing relationships and providing assistance

Social workers develop relationships with patients and families, and work from a patient-centered perspective. They provide assistance to caregivers who are experiencing stress.

Community involvement and awareness

Social Workers cultivate and maintain relationships with agencies in the community, and know which resources will be most beneficial. They identify appropriate community resources, and help patients to access those services.
